Dr. Vijaykumar Harbishettar, Psychiatrist, Advisor, Samvedna Care explained the importance of correct diagnosis of Dementia, prior to starting any treatment.&nbsp;<\/strong><\/p>\n\n\n\n<!--more-->\n\n\n\n<p>The early diagnostic evaluation of patients with dementia-like symptoms is essential to identify patients with cognitive decline who may actually have underlying conditions that can be treated and reversed. Dementia&nbsp;is a collective term used to describe various symptoms of cognitive deterioration, including a decline in memory functioning, difficulties with orientation, judgement and planning. <a href=\"https:\/\/www.samvednacare.com\/blog\/early-symptoms-to-detect-dementia-in-elderly-people\/\">Symptoms of Dementia<\/a> are best understood by \u2018ABC changes\u2019 in functioning, i.e., <strong>A \u2013 Changes in Activities of Daily Living and B &#8211; Behavioral and Psychological changes and C &#8211; Cognitive changes.<\/strong><\/p>\n\n\n\n<p>Making a diagnosis and providing treatment requires a lot of skill and often involves a multidisciplinary team. In a clinical setting to diagnose, the condition an expert goes through the following steps:&nbsp;&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>History taking from reliable family members<\/li>\n\n\n\n<li>Cognitive assessment<\/li>\n\n\n\n<li>Assessment of independence in\u00a0 activities of daily living<\/li>\n\n\n\n<li>Assessment of behavioral problems<\/li>\n\n\n\n<li>Assessment of caregiver stress and\u00a0 burden<\/li>\n\n\n\n<li>Other clinical investigations\u00a0<\/li>\n<\/ul>\n\n\n\n<p>Cognitive assessments and scans often help the expert understand the extent of deterioration in cognitive functioning and whether this is due to dementia. Sometimes dementia-like symptoms and ABC changes can occur due to other treatable conditions. These may include medical conditions like infections, space-occupying lesions, normal pressure hydrocephalus, delirium, or even depression.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Here are some conditions which produce dementia-like symptoms but can be improved with treatment:<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Nutritional deficiencies, in particular vitamin B12 deficiency\u00a0<\/li>\n\n\n\n<li>Sodium imbalance which can cause confusion and delirium\u00a0<\/li>\n\n\n\n<li>Metabolic syndromes which remain uncontrolled like hyperthyroidism, diabetes and liver disease\u00a0<\/li>\n\n\n\n<li>Undetected infections such as urinary tract infections which are very common in older adults\u00a0<\/li>\n\n\n\n<li>Hearing impairment where a person can appear confused and forgetful\u00a0 due to their inability to hear properly\u00a0<\/li>\n<\/ul>\n\n\n\n<p><strong>Delirium<\/strong> is often confused with dementia but often has an underlying cause which can be treated There are four points that distinguish delirium from dementia and these are :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Delirium has a sudden onset unlike dementia where the changes are slow and often go unnoticed in the initial stages<\/li>\n\n\n\n<li>The course of Delirium is fluctuating but in dementia there is a progressive worsening<\/li>\n\n\n\n<li>Orientation is impaired in delirium but in the initial stages of dementia, orientation is intact<\/li>\n\n\n\n<li>Sun downing is very common in delirium\u00a0<\/li>\n<\/ul>\n\n\n\n<p>Since Delirium is reversible and can be treated it is critical to identify the underlying cause early.&nbsp;<\/p>\n\n\n\n<p><strong>Pseudodementia<\/strong> is a situation where a person who has depression also has a cognitive impairment that looks like dementia. A person with dementia can also have depression therefore there is an overlap between the two. The differences between dementia and pseudodementia are:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Dementia has a slow\/ gradual onset whereas pseudodementia has a rapid onset<\/li>\n\n\n\n<li>Both the conditions can affect mood but in dementia, there are periods of positive mood. In pseudodementia, there is a constant depressive mood and negativism is mainly observed in such patients.\u00a0<\/li>\n\n\n\n<li>In dementia there is a gradual cognitive decline, unlike pseudodementia where fluctuations in cognitive impairment can be seen.<\/li>\n<\/ul>\n\n\n\n<p>The timely recognition and treatment of depression in elderly is important to differentiate from dementia and improve quality of life.<\/p>\n\n\n\n<p><strong>Reversible dementia and pseudodementia<\/strong> are often successfully treated; however, symptoms including cognitive impairment typically do not go away immediately. Both medications and psychotherapy techniques may be required before a noticeable decrease in symptoms is observed.
